Whether people have a right to control their own death has become a topic of increasing interest to everyone involved – governments that try to impose their will on individuals, advocates on both sides of the question, and those most directly affected, the terminally ill. This book, inspired by the Compassion in Dying Federation, looks at the issue personally, from the standpoint of the dying and those directly involved in the process. Editor Barbara Coombs Lee highlights stories of individuals and their graceful release into death that can happen when people are given a choice. But there are also powerful accounts by family members, friends, and religious advisers who respected and supported that choice – including those who opted for physician-assisted death. This publication coincides with the 10th anniversary of the Compassion in Dying Federation.

The patients' personal experiences fall against the backdrop of the Death with Dignity Law's origins, challenges, and opposition by right-to-life activists and U.S. Atty. Gen. John Ashcroft. These stories ultimately make us aware of the care terminally ill patients receive and argue for the right to die peacefully in the presence of loved ones rather than commit a violent suicide or allow family, pharmacists, and physicians to face prosecution. Clinical psychologist Riley (The Defiant Child) tackles a difficult subject with aplomb in this keenly insightful guide for parents. "For children and adolescents, the depressive state makes them feel like astronauts whose tethers have been cut, and they are drifting in space," he writes. He not only explains how to identify the symptoms of depression and the "negative beliefs" "no one will ever like me," "I am made of inferior stuff," "death is an option," and so on that are so often at its core, but also provides tools for what he terms "Planning the Rescue Mission." Ten brief but thorough chapters cover the most common negative beliefs that trigger depression and such companion problems as substance abuse, prematurely intense relationships (what he calls "miniature marriages") that blindside adolescents when they come to an end, and suicide. Riley illustrates each point with case studies that offer readers a chance to learn from his dialogues with his patients. He suggests strategies for everything from closing "the physical distance between you and your child" in order to soothe and encourage them, to some particular lines of questioning for uncovering negative beliefs, to the crucial, often overlooked act of listening ("your ears cannot be fully open until your mouth is fully shut"). He also discusses when to seek professional help and how to step in swiftly and effectively in the case of a suicidal child. Children and teens are increasingly being diagnosed as depressive--perhaps because modern life is becoming more challenging or possibly because treatments that have proven effective for adults also work well for children. A staff physician with the Division of Adolescent Medicine at Toronto's Hospital for Sick Children, Kaufman presents a good primer on treating teen depression. All of the major topics are covered in clear-cut language: how to find a therapist, possible treatments (including therapy and drug treatments, as well as alternative remedies), suicide prevention, and treatment of related substance abuse and anxiety disorders. Recommended if your library doesn't own Gerald D. Oster and Sarah S. Montgomery's Helping Your Depressed Teenager: A Guide for Parents and Caregivers (Wiley, 1994). The Depressed Child, on the other hand, is not recommended. Riley, a clinical psychologist, offers some interesting suggestions here and there (e.g., the importance of hugging a depressed child), but his idea that a parent should undertake to "rescue" a child who is suicidal or hallucinating is irresponsible. Such kids need professional help. David G. Fassler and Lynne S. Karp writes well, addressing psychological, chemical, and cultural perspectives, with much credit to C. Wright Mills, Erving Goffman, and Arthur Kleinman. Many psychiatrists would agree that too little attention is paid to the nature of the pain and the impact of social context on our definitions of normality and treatment. "Self-help" comes under fire, too, as shallow ideology in a time of advancing anomie. A careful, honest writer, Karp has produced a classic equal to William Styron's Darkness Visible (LJ 8/90) and Clifford Beers's A Mind That Found Itself (1908). Gambling is an underrecognized problem from both a clinical and a public health point of view. It is not uncommon for an adolescent or adult to participate in one form of gambling or another on a regular basis, be it the lottery, card playing for money, sports wagering, or gambling on electronic gaming devices. Nor is it uncommon for such participation to reach excessive or destructive proportions, with negative effects on the individual's psychological, social, economic, and family life. The results of the National Research Council's 2000 review of empirical studies suggest that 85% of adolescents report having gambled during their lifetime, with 73% of adolescents reporting having gambled in the past year. Despite gains in knowledge concerning the correlates and risk factors associated with severe gambling problems among youth during the past ten years, a general lack of public and parental awareness exists. Assessment: To date, there have been numerous articles, journals, and books produced to address the issues that patients, caregivers, and clinicians face at the end of life. The overlapping of information can be seen however in most cases as unavoidable. This book is well written and accurately reflects the state of end-of-life care in America.

A reminder that caregivers can consult spiritual leaders would have been sufficient. Interactive activities, interviews with relevant experts, and poetry provide a useful framework for working through this emotionally charged topic. These alarming developments have caused reactions ranging from profound grief to extreme anger in African-American communities, yet the organized political reaction has remained remarkably restrained.
The Boundaries of Blackness is the first full-scale exploration of the social, political, and cultural impact of AIDS on the African-American community. Informed by interviews with activists, ministers, public officials, and people with AIDS, Cathy Cohen unflinchingly brings to light how the epidemic fractured, rather than united, the black community. She traces how the disease separated blacks along different fault lines and analyzes the ensuing struggles and debates.
More broadly, Cohen analyzes how other cross-cutting issues—of class, gender, and sexuality—challenge accepted ideas of who belongs in the community. Such issues, she predicts, will increasingly occupy the political agendas of black organizations and institutions and can lead to either greater inclusiveness or further divisiveness.
The Boundaries of Blackness, by examining the response of a changing community to an issue laced with stigma, has much to teach us about oppression, resistance, and marginalization. Yale professor Cohen combines rigorous research and fresh sociological insights to build her argument that a black political agenda based solely on race promotes exclusionary practices. Cohen tracked responses to AIDS by black civic and church leaders and media in New York City (where, since 1990, AIDS has infected more blacks than any other racial or ethnic group), finding that they have espoused an understanding of racial identity that privileges middle-class, heterosexual males, while using code words "to designate who was expendable." Starting at the beginning of the AIDS crisis, she compares coverage by network television news and the New York Times with that of black newspapers and magazines. Cohen attributes the failure of black media to focus on AIDS at the beginning of the epidemic to homophobia, classism and sexism, resulting in the extreme stigmatization of the most disempowered members of black communities. She finds that in the 1980s, the black political response to AIDS came largely from black lesbians and gays. In recent years, women and children of color have come to be most at risk, while the black media focuses on alternative treatments and new heterosexual dating patterns in response to AIDS.
